Mission Statement: To help protect the health, safety and welfare of the dental consumer public by providing accurate, in depth personal and professional background information on licensure applicants to state licensing agencies and professional health care organizations.

Professional Background Information Services (PIBIS) is a non-profit organization whose primary purpose is to provide a centralized, uniformed process for state licensing authorities to obtain a verified, primary source record of a dentist's or dental hygienist's credentials. NOTE: PBIS provides credentialing services only and is not involved with the issuance of dental/dental hygiene licenses. Only state licensing authorities provide licenses. Furthermore, licensing authorities may require other verifications and/or information and credentials you possess in addition to those verified by PBIS.

There are two levels of services available - Level II and Level III. Contact the agencey where you are seeking licensure to determine the required level of service. Fees are as follows:

  • Level II

    Dental with License

    $500.00

    Level II

    Dental Hygiene with License

    $400.00

    Level II

    Dental or Dental Hygiene Unlicensed/New Graduate

    $350.00

    Level III

    Dental or Dental Hygiene

    $300.00

Note: An applicant who requests multiple states at the same time will be charged a reduced fee of two hundred dollars ($200.00) per each additional state which PBIS is currently affiliated with. Please contact the PBIS office at 602-861-5867 for further instructions.

Refunds: A significant portion of application processing occurs immediately after applications are received; therefore, in order to receive a refund, you must submit a written cancellation request by mail or fax and it must be received by PBIS within 48 hours from the time PBIS receives your application.
